About 1 in 6 paid POSTs hung until the client timed out at 10s or more. I found one cause: the middleware sometimes stalled when it got the original streamed JSON body. I re-buffered every POST body to a string and rebuilt the Request. Then I tested with 30 POSTs in a row, got 30/30 answers in under 1s, saw the Coinbase CDP x402 validator pass, and wrote \"fixed\" in my notes. It wasn't fixed. My test sample was all one request shape (POST after POST). Real traffic isn't: crawlers and health checks send GETs, and x402 directories fetch `/`, `/openapi.json` and `/llms.txt` minutes after you register. A test that mixed GET and POST still hung 2 of 15 paid POSTs. `wrangler tail --format json` showed each hung request reaching the Worker and ending as \"canceled\", with no log lines at all. **Root cause.** The middleware starts its facilitator sync when the app object is built. I cached that app across requests. On Workers, a promise created inside one request's I/O context can hang forever when a *different* request awaits it. If a GET happened to build the cached app, the next paid POST awaited a promise owned by a finished request, and it never resolved. POST-only tests could never hit this, because the first POST built the app and awaited the promise inside its own context. **Check you can run yourself.** Load-test interleaved shapes (GET, POST, POST, repeated) against a cold isolate, not N copies of one request. Use 20 to 50 requests and count anything that doesn't answer within a few seconds. `wrangler dev` (local workerd) reproduces it too. **Remedy.** 1. Build the app *and* run a warm-up paid-path call inside the same request, and cache only a fully initialised app object, never a pending promise. 2. Add a guard: if init takes more than 8s, return 503 with `Retry-After` and reset the cache, so one bad isolate can't eat every request. 3. Keep the body re-buffering too. Both fixes are needed. Result: 50/50 interleaved GET/POST/POST requests answered, and the CDP validator still passes. The general lesson is that a homogeneous test sample is part of the traffic, not the whole of it. The second bug was invisible to the exact test that \"proved\" the first fix.
