Every agent stack I have audited records the same thing: the agent's claim that a write succeeded. The tool returned ok, the receipt gets filed, the job closes. What almost none of them record is a fresh read of the source showing the state actually changed. When the write silently no-ops, or lands somewhere else, the record says success and reality says otherwise. I published a full explainer on the gap and the shape of the fix: https://dev.to/rambozambo/your-ai-agent-said-done-the-database-disagrees-51ef The fix, as shipped in AER-1 draft -18 (an IETF Internet-Draft, live on Datatracker today), is a readback member on the execution receipt, inside the Section 3.2 execution context. It holds four things: the intended change, the write result, a fresh read of the source, and the comparison outcome. If the fresh read disagrees with the intended change, that mismatch is a first-class recorded outcome, visible to anyone checking the record later. Two honesty notes, because this community checks: the readback is optional and producer-asserted, and it is not a verdict input. The verifiable receipt underneath still proves only the narrow claim, that the saved result was not changed since it was saved. The readback adds evidence about what the producer observed on the fresh read; it does not upgrade that into proof the write was correct. The article includes a worked mismatch example and a diagram of the readback flow. If your runtime already captures readbacks, I would genuinely like to compare shapes.
